Terms of Service

Welcome to the virtual practice of Raj Patel, MD ("Practice"). By accessing our website, portal, or enrolling in our Direct Primary Care (DPC) membership, you agree to these Terms of Service.

1. Scope of Practice & Patient Eligibility

• Age Limitation: This practice strictly provides internal medicine and primary care services to adults aged 18 to 64. We do not accept or treat individuals under the age of 18 or aged 65 and older.
• Geographic Restriction: Services are exclusively available to individuals who are physically located within the State of Washington at the exact time of the telehealth encounter.

2. Direct Primary Care Membership Model

This Practice operates on a Direct Primary Care model. This is a monthly membership agreement between you and your physician.

• Not Insurance: You acknowledge that this membership agreement is NOT health insurance and does not meet individual mandate requirements under state or federal law.
• Fees: Monthly membership fees are billed automatically and are required to maintain access to virtual appointments, messaging, and clinical support.

3. Account Termination & User Conduct

We reserve the right to terminate patient memberships for non-payment, failure to maintain physical residency/location in Washington State during encounters, aging out of the service window (turning 65), or abusive behavior toward clinical staff.
